D.Towner, and was honorably discharged at Louisville, state ofKentucky on the 10th day of July 1865. That his personaldescription is as follows: Age 50 years; height 5 feet 3 inches:complexion dark; eyes Grey; hair Dark. That while a member ofthe organization aforesaid, in the service and in the line ofhis duty, at or near a place called Birds Point, state ofMissouri, on or about _____day of Fall 1861. 1. Contractedchronic darrhora alternating with chronic constipation andresulting in (unreadable word) of digestive organs rectum andpiles caused by impure water and improper diet and other thingsincedent to army life. Was treated in camp by Regimental Surgeonfor about ten week. 2. Also while in the line of duty at ornear Donaldson State of Kentucky on or about the 16th of Feby1862 contracted chronic rhumatism and resulting disease ofheart. 3. Also while in the line of duty at or near CorinthState of Missip in June 1862 contracted malaria at ( unreadableword ) and resulting disease of liver and spleen. (unreadableword ) exposure was treated for said disabilities (2 and 3 ) incamp for several months by Regimental Surgeon. Claimant desiresto be ordered before the board of examining surgeons at NewtonKansas. That he has not been employed in the military or navalservice otherwise than as stated above. Since leaving theservice he has resided in Iowa and Kansas and his occupation hasbeen that of a Farmer, that prior to his entry into the serviceabove named, he was a man of good, sound, physical health, beingwhen he enrolled a Farmer. That he is now physically disabledfrom obtaining his subsistance by manual labor by reason of hisinjuries above described, received in the service of the UnitedStates; and he therefore makes this declaration for the purposeof being placed on the invalid pension roll of the UnitedStates. He hereby appoints, with full power of substition andrevocation, O.E. Howe, Winfield, Kansas, his true and lawfulattorney, to prosecute his claim. That he has not received orapplied for a pension.
